1. Company Profile, organization chart and year of establishment including

the following document copy

1.1 Business Registration Certificate of the applicant

1.2 Certificate of General Registered Building Contractor issued by

Building Department.

2. Relevant job reference on building repair works and repartitioning works

for shopping malls or commercial buildings in the past 5 years (based on

commencement date)

Please give details on client, name of shopping centres, address,

description of contract works, contract value ($1M to $15M and above

$15M), contract type (term contract or one-off work), contract

commencement date, completion date and client's appreciation letter.

3. Detailed information on your management and project team:

3.1 Structure of Management Team with C.V. indicating their working

experience, qualification and relevant training.

3.2 Structure of Project Team including Construction/Project Manager,

Qualified Quantity Surveyors, Site Agent and Foreman with C.V.

indicating their working experience, qualification and relevant

training.

3.3 No. of directly employed technical staff / site foreman with their

respective trades.

4. Sub-contractors Management Plan with the following details :-

7. Audited financial statements for the last three years

Only qualified contractors will be invited to tender.

You should not apply for prequalification as a tenderer for this Contract independently if you intend to act as a supplier, sub-contractor or sub-consultant for another company who will apply for the prequalification, or if you are a partner of a joint venture established to seek prequalification for this Contract.

Companies having linkage to each other, e.g. subsidiaries, holding or sister companies, shall not apply for prequalification as a tenderer for the same Contract, whether on their own or as a partner of a joint venture. “Sister companies” shall mean all companies which are subsidiaries of or otherwise belonging to the same holding company. The existence of a holding-subsidiary relationship shall be determined in accordance with the provisions in Sections 13 to 15 of the Companies Ordinance (Cap. 622) of Hong Kong. Companies having linkage to each other shall determine which company will apply for prequalification.
