Applicant must download the application form to his/her computer first and print the blank application form out directly or fill in the information required on the form before printing out.
Completed application form must be endorsed by the applicant's school and submitted to any MTR Customer Service Centre (except Airport Express stations, Lo Wu, Lok Ma Chau, Racecourse, Disneyland Resort, Sunny Bay, Wong Chuk Hang, Lei Tung, South Horizons, Hin Keng, Kai Tak, Sung Wong Toi and To Kwa Wan stations).
